Ophthalmic Preventive Maintenance
Following is a Preventive Maintenance checklist which decribes all the services of a typical ophthalmic PM.
Ophthalmic Chair
Check all operations, mechanical, hydraulic and electrical. Make adjustments to arms, headrest, tilt and swivel locks.
Inspect base for hydraulic leakage.
Inspect upholstery for tears and rips.
Clean all surfaces.
Ophthalmic Stand
Check all operations, mechanical and electrical.
Make adjustments to Phoroptor, Keratometer & Slit Lamp arms or tables.
Inspect Overhead lamp for broken joints and stops. Careful attention paid to wiring and lamp shade rotation stops.
Check charging well operation, BIO & OH lamp dimmer functions.
Make sure safety screw is on Phoroptor arm.
Clean all surfaces.
Slit Lamp
Check all operations, mechanical, electrical & optical.
Ensure that bearing tube/ rollers operate correctly.
Check & adjust elevation and swivel locks.
Check for collimation of stereo microscope body.
Check operation & collimation of mag changer.
Check condition of lamp socket & housing.
Check operation & condition of power supply and all wiring.
Check alignment of Applanation Tonometer.
Check table levelness.
Check Hruby lens.
Check & adjust slit & filter and aperture operations.
Clean all surfaces.
Phoroptor
Loaner Phoroptor is left and customer unit brought to shop.
Unit is disassembled.
All detents are cleaned and lubricated.
Thrust washers and bearings are de-greased, cleaned and lubricated.
Geneva assemblies are cleaned and lubricated.
Cylinder dials are cleaned. Proper operation of moon & star is checked.
Turret assemblies are disassembled, cleaned and rebuilt.
Rotation and swivel assemblies are adjusted.
Pd assembly is cleaned and adjusted.
Rotary prisms are checked for proper operation & repaired if needed.
Instrument is reassembled.
Proper axis of all cylinder lenses is verified on projection fixture.
Proper calibration of rotary prisms is verified on projection fixture.
Cross cylinder alignment and operation is verified.
Proper tensions set to allow correct Turret operation.
All lenses are cleaned.
All outer surfaces are cleaned.
Keratometer
Check all operations, mechanical, electrical and optical.
Check lamp housing, wiring, socket, switch and reflector.
Check all elevation rotation and lock mechanisms.
Check meyers and other internal optical components.
Check eyepiece assembly.
Clean all optics and mechanical assemblies.
Verify calibration with Lenscometer system.
Check Occluder. Verify doctor wants it before replacing it if missing.
Spacer Tray/Adapter
Ensure instrument is properly secured & level.
Check for excess wear on end of tray peg.
Projector
Check all mechanical, electrical and optical assemblies.
Inspect lamp socket and switch for wear or burning.
Align lamp socket and bulb.
Check chimney assembly.
Check sprocket and gears for correct slide movement.
Check Cross slide. Make sure correct side faces bulb to prevent melting.
Check optics in projection tubes.
Check Sizing with calibration chart.
Check housing spring lock.
Check slides for cracks & clean.
Verify presence of other slides.
Check Fixation assembly.
Projector Mount
Inspect and make sure all locks are secure.
Verify mount is correctly secured to wall.
Verify floor or table mounts are correctly placed.
POC Screen & Mirror
Verify correct placement and alignment of all mirror & screen.
Verify items are securely mounted to wall.
Verify mirror adjustments are securely locked in place.
Hand Instruments
Check operations on all.
Verify correct operation of rheostats on handles.
Verify correct operation of scope heads.
Clean external optics.
When needed leave loaner head & disassemble at shop.
Trial Lens Set
Verify condition of lenses.
Tighten loose screws on handles when apparent.
Inspect condition of Trial Lens Case.
Trial Frame
Check all mechanical movements and assemblies.
Clean frame.
Applanation Tonometer
Check operation of instrument.
Verify correct detents in prism arm.
Check calibration with Haag Streit Tonometer fixture.
Clean all surfaces
Tonometer Mount
Inspect instrument.
Ensure unit is properly aligned with Slit Lamp optics.
Ensure all mounting screws are tight.
Lensmeter
Check all electrical, mechanical & optical Assemblies.
Inspect eyepiece and internal reticles.
Check table operation.
Check marking pen operation.
Inspect target for correct alignment.
Check optical carriage performance.
Check lamp house, socket & switch.
Ensure correct lamp is in housing to prevent melting.
Check power and axis dials for correct movement.
Check swivel mechanism for correct operation.
Verify correct calibration using Gauge Master or other certified lens set.
Clean all optics and other surfaces.
BIO
Check all operations, mechanical, electrical and optical.
Check socket and lamp housing.
Check headband assembly.
Verify collimation of BIO Viewer.
Check power supply & wiring.
Ensure BIO hanger is secure.
Other items
Verify if Occluders, lenses, roto-card and other items called out on sheets are present and what condition they are in. Clean items.
HIGH TECH ROOM
NCT
Check all operations.
Check elevation, carriage movement & safety locks.
Clean all optics.
Check printer operation (if applicable).
Check video operation (if applicable).
Replace target lamp.
Clean internal optics.
Check high intensity operation.
Check forehead & Chin rest assemblies.
Check wiring and power supply.
Check functions (demo / override).
Check left right monitor operation.
Check readings with non certified test eye.
AutoRefractor
Check all operations.
Check video performance.
Check printer operation.
Check elevation and carriage movement.
Check safety locks.
Check Chin Rest and Forehead rests.
Check left right monitor operation.
Verify accuracy of readings with Topcon Test Eye fixture.
Fundus Camera
Check front lens for scratches or mar in coating.
Check all electrical connections and assemblies.
Check Polaroid camera operation.
Check 35mm & video camera (if present).
Check mechanical operations.
Check flash & internal fixation.
Check carriage movement.
Have technician take photo to verify performance.
Power Tables
Check all electrical connections.
Check proper operation of up/down and limit switches.
Check condition of table top.
Check casters.
